Usefulness of Screening Breast Sonography in Asymptomatic Women with Mammographically Dense Breasts

Abstract
Purpose: To evaluate the usefulness of screening breast sonography in asymptomatic women with mammographically
dense (Breast Imaging Reporting and Data System [BI-RADS] density category 3 or 4) breast tissue.

Materials and Methods: Between January 1998 and December 2002, 107,766 women were performed with screening mammography in our institution. Of 107,766 women, 26,458 women showed BI-RADS pattern 3 or 4 breast composition. Finally, 576 women with negative findings at clinical examination, negative mammographic results and dense breast tissue were evaluated with bilateral whole breast screening sonography. Abnormal findings on breast sonography were either followed-up or diagnosed by US-guided fine needle aspiration biopsy, core needle biopsy, mammotome biopsy or surgical excisional biopsy.

Results: Among the 576 women (age range; 22-70 years, mean; 42.8 years) evaluated with bilateral whole breast screening sonography, 405 sonography showed normal finding and 171 sonography showed abnormality including cysts or benign looking lesions (n=145) and indeterminate nodules (n=26). 73 biopsies were performed in 67 patients and follow-up breast sonography data was available in 125 patients. The results of 73 biopsies (benign-appearing nodule=47, indeterminate nodule=26) were benign in 71 and malignancy in 2 lesions. Two breast cancers (1 DCIS and 1 invasive ductal carcinoma) were detected (cancer detection rate, 0.35%).

Conclusion: Bilateral screening breast sonography in the evaluation of asymptomatic women with mammographically
dense breasts may be beneficial in detecting early breast cancer not detected with mammography or clinical examination.
